Our clinical team is comprised of:

Sessional input from an experienced Consultant Psychiatrist

Our clinical team provide a detailed assessment and proposed package of care prior to an individual’s transition to any of our services. The detailed care plan covers a comprehensive range of domains of daily living and considers mental, physical, occupational and social needs. Care plans are shared with individuals in regular key worker sessions and individuals are encouraged to participate fully in the care planning process.

The clinical team meet to review each individual’s care plan, risk assessment, progress and their expressed views gained through key working sessions on a regular basis, and at least quarterly. Minutes of this meeting are produced as a report for referrers/commissions and provide a useful record of the individual’s progress. The clinical team is also keen to attend CPAs/CTPs/Professional meetings arranged by the individual’s care team in order to provide an up-to-date account of the care and support provided and agree shared goals with services.
